1. All MEMBERSHIP HOLD - FEE PERIOD EXTENSION must be via this form. Please do not send requests via email or text.

2. Fees have been calculated to take into account the scheduled breaks when our centre is closed i.e. grading days, competition days and public holidays. This means your payments continue on an ongoing basis; they are not dependent on the centre being open. For membership hold requests that include non-training days, only the number of training days will be extended.

3. Please check your junk mail if you did not receive email confirmation of your request.

4. ADMINISTRATIVE FEES apply to membership hold requests :
$25 for 1st week +

$2 per week after the 1st week (rounded up to the nearest week).

5. Minimum 7 days of membership hold request.

6. All fees must be current before membership hold applies.

7. An invoice for the administrative fees will be emailed upon receipt of membership hold request. Fee period extension will be processed only upon receipt of administrative fees.

8. Medical reasons or injury - waiver of administrative fees
- Medical reasons or injury must be supplied with a medical certificate provided by a medical practitioner for waiver of administrative fees.

- Duration of membership hold will be subject to what is stipulated by the medical certificate (minimum 7 days)
- Standard administrative fees will apply for absent period not covered by the medical certificate

- Medical certificate must be emailed to info@weiwutkd.com.au

WARRIORS

COMPULSORY minimum attendance requirement :

12 weeks (84 days) prior to each competition : 85% attendance

12 months prior to each competition : 85% attendance
